Upon my demise while a member of the Society, I instruct that all amounts due to me, less any debts owed to the Society, be paid to the nominee(s) named above according to the allocation percentages indicated. I understand that I may amend my nomination by submitting a new nomination form.

I, the Applicant hereby apply for membership in Loita Traders Savings and Credit Co-operative Society Limited (Loita Traders SACCO) and certify that the information provided herein is true and accurate to the best of my knowledge. I agree to abide by the SACCO’s By-Laws, rules, and regulations, and any amendments thereto, as prescribed under the Co-operative Societies Act (Cap 490) and by the Commissioner for Co-operative Development. I hereby authorize Loita Traders SACCO to verify any information provided, including my KRA details, employer, or other references, for purposes of membership and regulatory compliance. I consent to the collection, verification, and processing of my personal data by Loita Traders SACCO in accordance with its Privacy Policy and the Data Protection Act, 2019 (Laws of Kenya). I further authorize Loita Traders SACCO to access my credit information from and submit my credit information to licensed Credit Reference Bureaus (CRBs) and other credit providers for the purposes of credit assessment, monitoring, and compliance with the Credit Reference Bureau Regulations, 2020. This consent shall remain valid for as long as I maintain a relationship with the SACCO or have any outstanding financial obligations.
